In Western society, a common fixture of etiquette is the concept of holding doors open for other people who are about to come through. In this comic, a man is depicted as doing this for a woman; however, rather than simply acknowledging her politely, as would be the norm, he subverts the expected conduct of the situation by loudly and rudely declaring that she is ugly, and should be grateful that he did so. This is considered extremely bad manners in pretty much any real-life situation, and it frames the man's motivations as being less wanting to do something considerate for its own sake and more due to trying to impress attractive women. This is further reinforced during the votey when he is depicted as believing that this minor inconvenience constitutes suffering on his part (when in reality any reasonable person would not consider this as such).
This comic is one of many with the recurring theme of, as Zack puts it, "men being douchebags" (see also: 2006-07-19), which formed the basis of a lot of punchlines in Zach's earliest work. Zach himself lampshades this concept in the alt-text, which, in turn, forms another recurring theme of him directly or indirectly expressing regret for relying on this type of shock-oriented and/or sexist humor a lot during this phase of his career(see also: 2006-07-19).
