A red ball buries itself in the neck of a man out running, swallowed by his skin. 
The man is seen in his socks and underwear, then with a deteriorating fever.
His face shows anger/fear/confusion/pain as his stomach appears to move on its own from the inside.
In the final scene, three miniature clowns are seen bursting from his stomach.
(This is likely a reference to the "chest burst" scene in the film Alien)
This is also part of Zach's larger oeuvre describing Clowns as a species.
